Patients need to bring the following items to their first podiatry appointment, as applicable to their individual circumstance:
- New Patient Details form – if you completed this form at home
- All referral paperwork
- Private health insurance card
- Department of Veterans Affairs or Medicare card
- Pension card.
Our receptionist may ask you to bring other items, depending on the reason for your appointment. These items could include x-ray films, medical reports, and shoes.
